ABSTRACT

Objective To evaluate the changes of brain metabolism with 18F-fluorodeoxyglucose (FDG) PET/CT in patients with disorders of consciousness (DOC).Methods This retrospective study,from January 2007 to October 2016,included 40 patients (26 males,14 females,age range:17-73 years)in the vegetative state (VS),12 patients (11 males,1 female,age range:25-53 years) in the minimally conscious state (MCS),and 11 patients (10 males,1 female,age range:12-68 years) in the state of recovering from DOC.All patients underwent 18F-FDG PET/CT imaging.The standardized uptake value (SUV) of multiple brain areas among 3 groups of patients was calculated and compared.One-way analysis of variance was used for data analysis.Results The SUV in different encephalic regions among 3 groups were significantly different (F values:6.214-13.642,all P<0.01) except for mesencephalon.Compared with MCS group,the SUV of cerebral cortex of VS group was lower (t values:2.263-3.548,all P<0.05).Compared with the recovered group,the SUV of cerebral cortex and cerebellum of VS group was lower (t values:1.299-5.136,all P<0.05).Compared with the recovered group,the SUV of parietal lobe,temporal lobe,occipital lobe,thalamus and cerebellum of MCS group was lower (t values:1.962-2.841,all P<0.05).Conclusion 18F-FDG PET/CT may be significant in evaluating brain function of DOC patients.

ABSTRACT

Objective To prepare PET molecular probe folate-NOTA-Al18F(18F-FNA) and to explore its feasibility as an imaging agent in folate receptor positive KB tumor.Methods 18F-FNA was prepared by the method of aluminum fluoride coordination labeling, and the effect of phase transfer catalyst K2.2.2 on the labeling yield was evaluated.Biological distribution was carried out at 10, 30 and 90 min after injection of 3.7 MBq 18F-FNA in nude mice (n=20) xenografted with KB tumor, and the radioactive uptake (%ID/g) and T/NT ratios were then calculated in different organs or tissues.MicroPET imaging was performed at 40 min after injection of 18F-FNA (3.7 MBq).Results The labeling yield of 18F-FNA increased with the presence of K2.2.2.The radiochemical yield was above 98%.The radiochemical purity was above 99%, and still above 98% after maintained in PBS and FBS at 37 ℃ for 4 h.The biodistribution showed that the blood clearance of the probe was slow, and the uptakes in kidneys and tumor which overexpressed folate receptor were significantly high ((5.12±0.58) %ID/g and (1.37±0.20) %ID/g).The high radioactive uptake was observed in KB xenografted mice using microPET imaging.Conclusions The labeling yield of 18F-FNA could increase with the presence of K2.2.2.Furthermore, the encouraging biological distribution and microPET imaging results indicate that 18F-FNA may be a candidate for PET imaging in targeting folate receptor.

ABSTRACT

Purpose To prepare 68Ga-NOTA-SPIO as PET/MRI dual mode imaging probe with high sensitivity and resolution,and further evaluate its in vitro and partly in vivo biological properties.Materials and Methods The precursor SPIO-PEG2000-NOTA was prepared and characterized.The precursor was radiolabeled by using one step method to prepare 68Ga-NOTA-SPIO as dual mode imaging probe.The labeling rate of the probe was determined by rapid thin-layer chromatography.Besides,the in vitro stability and lipid water partition coefficient of the probe were evaluated,and its biodistribution in normal mice was also observed.Results The precursor SPIO-PEG2000-NOTA with uniform dispersion and uniform particle size was prepared,and the dual mode probe 68Ga-NOTA-SPIO was synthesized.The labeling rate reached 99%,and the lipid water partition coefficient (Log P) was (-2.60±0.13).The radiochemical purity of the probe was higher than 95%,as it was incubated in the phosphate buffer and fetal bovine serum within 2 hours.The probe was mainly distributed in the liver and spleen of mice,and its clearance velocity in blood was fast.Conclusion The double mode probe 68Ga-NOTA-SPIO synthesized by one step method has high labeling rate with no need of purification,which has good physic-chemical properties and biocompatibility.The probe can be used in the further research of PET/MRI dual modality imaging.

ABSTRACT

Objective To evaluate the ability of a synthetic PET/NIRF probe,named 18F-PSVue643,on the apoptosis detection in animal models with the anti-cancer drugs therapy,and compare the advantages and disadvantages between PET and NIRF.Methods Cell apoptosis was detected by MTT and flow cytometry in vitro.Established U87MG glioblastoma xenograft tumors in nude mice were treated with retinol and paclitaxel for nine days (for PET imaging) or eleven days (for NIRF imaging) continuously.The uptake values were recorded by ROI and expressed as %ID/g.Results The apoptosis ratios in 10 and 100 nmol/L paclitaxel-treated groups were 7.4% and 7.5%,respectively,and the apoptosis ratio of the control group was 4.3%.The apoptosis could be well detected by both NIRF and PET imaging during the whole process of treatment.However,the amount of probe for PET imaging was only a half of that for optical imaging to get the same apoptosis visualization.Conclusion 18F-PSVue643 is suitable for NIRF and PET imaging in detection of apoptosis,and it may be a promising agent for further clinical studies.

ABSTRACT

We report a facile and effective method for radioiodine-labeled radiopharmaceuticals via copper (I)-catalyzed click chemistry route. In the novel radioiodination method, 5-iodo ((125/131)I)-1, 2, 3-triazoles were synthesized after a 24-h click reaction in organic solvent with a radiochemical yield of 13%. However, in the aqueous phase, the radiochemical yield of the conjugation radioiodine to RGD via click chemistry was 0. This suggested an exchange between hydrogen ion and iodine ion in aqueous phase so that no enough radioiodine was left to conjugate with RGD. We propose different mechanisms of Cu (I)-catalyzed cycloaddition of organic azides and 1-iodo-alkynes in organic phase and aqueous phase.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the value of (18)F-FDG PET/CT in the diagnosis of extrapulmonary tuberculosis (ET).</p><p><b>METHODS</b>The imaging data from 88 cases of ET undergoing (18)F-FDG PET/CT examinations between August, 2005 and May, 2011 were analyzed retrospectively.</p><p><b>RESULTS</b>All the 88 cases showed high (18)F-FDG uptake on PET imaging with a SUVmax ranging from 1.3 to 23.2. In 19 of the 88 cases, the extrapulmonary lesions were misdiagnosed as malignant neoplasms. Twenty-two patients were found to have phthisis. According to locations, the lesions were classified into 4 basic types, namely membranous tuberculosis (36 cases) with soft tissue thickening as the main manifestation (only 6 cases did not present with obvious thickening), lymphatic tuberculosis (44 cases) with lymph node enlargement (ill-defined edge in 22 cases, central necrosis in 3 cases, and ring-like enhancement in enhanced CT scan in 6 cases), osteoarticular tuberculosis ( 28 cases) with the main findings of osteolytic destruction with or without osteosclerosis (tuberculosis of the spine was characterized by disc damage and paravertebral cold abscess; only one case showed high FDG uptake without bone destruction), and organ tuberculosis (25 cases) involving the liver, spleen, pancreas, kidney and brain, where lesions of low density were detected in CT scan (3 cases showed ring-like enhancement). In cases of organ tuberculosis, nasopharyngeal and laryngeal tuberculosis showed soft tissue thickening, and intestinal tuberculosis presented with slight intestinal wall thickening (involving multiple segments in 2 cases). Of the 88 cases, 30 had two or more than two lesion types.</p><p><b>CONCLUSION</b>PET/CT can accurately demonstrate the morphology, extent of involvement and uptake activity of the ET lesions to assist the early diagnosis and treatment of ET. Enhanced CT scan also helps in the differential diagnosis of the lesions.</p>

ABSTRACT

Objective To analyze the pitfalls of 18F fluorodeoxyglucose (FDG) positron emission tomography/computer tomography (PET-CT) scan in the diagnosis of 60 patients of tuberculosis mimicking malignancy.Methods The study included 60 patients with PET-CT diagnosis of probable malignancy.Fifty patients were proved to be tuberculosis by pathological examinations and 10 were diagnosed by clinical followup.The images of whole body were acquired at 60 min after administration of 222-555 MBq 18F-FDG.The PET-CT imaging characteristics and clinical data,including lesion size,distribution,standardized uptake value (SUV) were retrospectively analyzed.After the whole body scan of PET-CT,each patient had a chest spiral CT scan for detailed observation of lung lesions.Contrast enhanced CT (CECT) was performed in 8 patients.Results (1)Thirty patients were misdiagnosed as lung cancer,14 patients as malignant lymphoma,6 patients as malignant mesothelioma,3 as intestine carcinoma,2 as bone malignancy,1 patient as hepatocarcinoma,spleen malignancy,ovarian cancer,laryngocarcinoma and nasopharyngeal carcinoma respectively.(2) 90.9% (20/22) of patients showed normal level of serum CEA and 100% (13/13) of patients showed normal level of CA199.Increasing serum CA125 was found in all patients (6/6) with activeTB patients accompanied with ascites,pleural fluid and (or) pericardial effusion.(3) 93.3% (28/30)active tuberculosis showed accumulated 18F-FDG which was incorrectly interpreted as malignancy.The most common sites of TB lymphadenopathy were bilateral cervical tissues,which was accounted for 85.7%(12/14).CECT revealed characteristics of peripheral enhancement and central necrosis in tubercular lymphadenopathy,which was 87.5% (7/8).Conclusions The diverse manifestations of TB on imaging and high uptake of 18F-FDG on PET imaging result in misdiagnosis of malignancy.It is important for radiologists and nuclear medicine physicians to identify the common imaging features and patterns of TB to make a correct diagnosis.Integration of reconstruction HR CT,PET-CT and lab examinations may improve the diagnostic accuracy.

ABSTRACT

Objective To evaluate the value of whole body 18F-FDG PET/CT in detecting small intestine adenocarcinoma (SIA).Methods A retrospective study of 18F-FDG PET/CT was performed on 29 cases (male 17,female 12) of SIA,21 cases of small intestine lymphoma (SIL) (male 15,female 6) and 10 cases of small intestine tuberculosis (SIT) (male 4,female 6).Visual and semi-quantitative methods (SUVmax) were used to summarize and analyse the 18F-FDG PET/CT resutls.One-way analysis of variance and x2 test were used to analyze the data.Results (1) 18F-FDG PET/CT for SIA showed a partially conglomerate pattern of hypermetabolic small bowel masses with nodular configurations.A typical SIL showed a partially annular abnormal growth with aggregated foci of radioactivity.SIT lesions were usually in form of stripes and/or nodules with high metabolic foci or lesions withskipped distribution.The SUVmax of SIA (8.44±3.82) was significantly lower than that of SIL (11.54±4.02; F=86.96,t=2.77,both P<0.01),but not significantly different when compared with SIT (8.61±2.99; t=0.11,P>0.05).(2) The incidence rates of peri-lesion lymph node enlargement in SIA,SIL and SIT were 72.41% (21/29),85.71% (18/21) and 70.00% (7/10),respectively (x2 =1.50,P>0.05).The SUVmax of peri-lesion lymph nodes in SIA (5.59±2.86) was significantly lower than that of SIL (11.10±5.72; F=56.56,t=3.85,both P<0.01),but was not significantly different when compared with SIT (5.63± 3.36; t =0.30,P> 0.05).The detection rate of PET/CT on pefi-lesion lymph node enlargement of SIA was higher than CT (41.38%,12/29; x2 =5.69,P<0.05).(3) The incidence rate of extra-intestinal metastases was 55.17% (16/29) in SIA,and the most common metastatic sites were liver,bone and adrenal gland.The incidence rate of extra-intestinal lesions was 66.67% (14/21) in SIL,most commonly presented as widespread multifocal nodal permeation.Extra abdominal tuberculous loci were found in 80.00% (8/10) of SIT.The incidence rates of extra-intestinal involvement were not significantly different among SIA,SIL and SIT (x2=2.13,P>0.05).(4) Among the 29 SIA cases,15 (51.72%) involved the ileum,8 (27.59%) jejunum,4 (13.79%) both jejunum and ileum,and 2 (6.90%) both jejunum and duodenum (x2 =18.16,P<0.01).(5) Among the 29 SIA cases,primary SIA was found in 8 cases (27.59%),double primary in 7 cases (24.14%) and metastatic SIA in 14 eases (48.28%).(6) Based on the clinical data,pathological results and clinical follow-up,the diagnosis of SIA by 18F-FDG PET/CT had a sensitivity and specificity of 93.10% (27/29) and 80.00% (8/10),respectively.Conclusions 18F-FDG PET/CT is useful for the differential diagnosis of S1A and for supporting the diagnosis of single primary,multi-primary and metastatic SIA.

ABSTRACT

Objective To study the relationship between the 18F-FDG PET/CT whole body imaging characteristics and pathologic features of small intestine primary malignant neoplasms.Methods A retrospective study was carried out on 18F-FDG PET/CT data of patients(n =44)with small intestine disease(25 malignant,19 benign).All results were proved by pathologiy,or diagnostic therapy and/or clinical follow-up.The intestinal wall thickness(IWT)and maxium standardized uptake value(SUVmax)were used to analyze the imaging features under different pathological conditions.Results(1)The incidence rate of local tumor formation for benign and malignant intestinal lesions was 31.6% and 84.0%(x2 =10.40,P < 0.01),respectively.The IWT was(0.75 ± 0.32)cm and(1.42 ± 0.20)cm(t =2.66,P < 0.01),respectively.The S UVmax was(5.17 ± 3.04)and(9.65 ± 5.48)(t =2.88,P < 0.01),respectively.The incidence rate of enlargement of mesenteric lymph nodes for benign and malignant intestinal lesions was 52.6% and 72.0%(x2 =1.01,P > 0.05),respectively.The SUVmax was(4.80 ± 1.91)and(7.00 ± 5.61)(t =1.11,P > 0.05),respectively.(2)The metastasis incidence rate of other organs for intestinal malignant lesions was 44.0%.The most common metastasis organs were liver(28.0%),distant lymph nodes(20.0%)and bone (16.0%).The most pathological type of intestinal malignant lesions were malignant lymphoma(44.0%),adenocarcinoma(32.0%).The IWT was(1.41 ± 0.59)cm and(1.36 ± 0.62)cm(t =0.18,P > 0.05),respectively.The SUVmax was(12.49 ± 7.00)and(7.93 ± 2.82)(t =7.55,P < 0.01),respectively.The metastasis incidence rate was 9.1% and 62.5 %(x2 =3.89,P < 0.05),respectively.(3)Taking the intestinal local tumor formation and SUVmax andnot less than 4.2 0 wall thickening,and / or metastasis as the main basis of the intestinal malignant lesions,the sensitivity,specificity and accuracy were 96.0%,94.7% and 95.4%,respectively.The main pathological type of false positive for '8F-FDG PET/CT imaging was intestinal tuberculosis,and false negative was signet(f)ing cell carcinoma.Conclusion 18F-FDG PET/CT imaging has a higher clinical diagnosis value in differentiating small intestine primary malignant carcinoma from benign ones.The size and SUVmax of perifocus lymph nodes could not point out its malignant degree.The 18F-FDG uptake of the lesions is correlated with pathological type.The IWT is not correlated with pathological type.

ABSTRACT

Purpose To investigate the diagnostic value of ~(18)F-flurorodeoxyglucose (~(18)F-FDG) PET/CT (positron emission tomography/computed tomography) on multiple myeloma.Materials and Method ~(18)FDG PET/CT imaging findings were retrospectively analyzed in 31cases with multiple myeloma proved by bone marrow aspiration or biopsy from August 2005 to June 2009. Results The PET/CT imaging characteristics were categorized into three types: ① osseous destruction (n=28): Osseous destruction with active metabolism was found in this type combined with no extramedullary organs involvement. The SUV_(max) ranged from 2.5 to 15.4, and the SUV_(arg) ranged from 2.0 to 11.3. ② Extramedullary organ involvement (n=2): Besides osseous destruction, extramedullary organs involvement were detected in this type demonstrating active metabolism in varying degrees. The SUV_(max) ranged from 2.2 to 8.3 and the SUV_(avg) ranged from 1.7 to 6.5. ③ Diffuse bone marrow infiltration (n=1): Diffuse hypermetabolism without osseous destruction was found in the whole body bone marrow with the SUV_(max) of 13.4 and SUV_(avg) of 8.8.Conclusions ~(18)FDG PET/CT can accurately demonstrate the morphology, extent and activity of multiple myeloma, thus facilitating the clinical staging, whereas multiple osteolytic osseous metastases and osseous lymphoma should be excluded for certain cases.

ABSTRACT

Objective To test image quality, accuracy of attenuation and scattering corrections of PET with NEMA NU2-2001. Methods By using NEMA IEC/2001 PET Phantom,along with the test methods recommended by NEMA NU2-2001, the PET image quality, accuracy of attenuation and scatter corrections were measured. Results Relative error in attenuation and scatter corrections were 0.25 (n=4) and 0.25 (n=8) respectively. Conclusion It's difficulty to prepare the phantom and process data. Preparing the phantom according to the radioactivity ratio strictly and processing data carefully even with many times, the exact result of the test can be obtained.

ABSTRACT

Background and purpose:Increased glucose metabolism is a characteristic of malignant tumors.This characteristic might be related to a facilitative glucose transporter(Glut-1) and the proliferating activities of tumors.This study was aimed to assess the relationship among the over-expression of facilitative glucose transporter(Glut-1) and proliferating cell nuclear antigen(PCNA) as well as the fluorine-18 fluorodeoxyglucose(18F-FDG) uptake of tumors in patients with nasopharyngeal carcinoma.Methods:Between March 2005 and August 2006,40 patients with nasopharyngeal carcinoma were imaged with 18F-FDG positron emission tomography(PET).Their maximum standard uptake values(SUVmax) were measured.The expression of Glut-1 and the proliferating cell nuclear antigen(PCNA) in the 40 cases were studied in paraffin sections by SP immunohistochemistry.Results:The 18F-FDG uptake of tumors of the 40 patients with nasopharyngeal carcinoma was 9.4?1.9(SUVmax).All 40 tumors tested Glut-1 positive and PCNA positive.The Glut-1 positive cells consisted of 45.18% of the tumor cell area,whereas the PCNA positive cells consisted of 36.18% of the tumor cell area.There were correlations between Glut-1 expression levels(r=0.369,P=0.019) and the tumors' 18F-FDG uptake but no correlations were found between PCNA expression level and the tumors' 18F-FDG uptake(r=0.135,P=0.407).Conclusion:Glut-1 over-expression correlates with 18F-FDG uptake whereas PCNA over-expression does not correlate with 18F-FDG uptake in patients with nasopharyngeal carcinoma.
